REAUTHORISATION beyond the initial 6 months requires:
   R1. Documented weight loss of at least 5 percent of baseline body weight at
       the time of the reauthorisation request, and
   R2. Continued participation in the lifestyle intervention.
   A member who has not achieved 5 percent loss is denied at reauthorisation
   under R1; this is not a pend.

EXCLUSIONS — deny if any apply:
   X1. Personal or family history of medullary thyroid carcinoma or multiple
       endocrine neoplasia type 2.
